Leave a message with student name and number.  Please include your name and relationship to student.
Reason for absence/early release/late arrival
Date and return phone number and a signature.
If you know ahead of time that your student has an appointment and will need to be dismissed early please send a note with your student or call the day before so that there will be less classroom interruptions.   

If you send a note be sure to have your student's name and number on the note and the date(s) they were absent/late or need to leave early.
